Internal Medicine Group (IMG), founded in 1953, is one of the longest running physician groups in Cheyenne, Wyoming. The practice is currently located in the modern medical office building adjacent to Cheyenne Regional Medical Center, Cheyenne’s hospital. This location offers convenient access to parking in the high rise parking garage and pass-through access to the hospital. The office hours are Monday through Friday between 7:30 a.m. and 5 p.m. Support staff is arranged into departments. The clinical staff boasts four RN’s, a phlebotomist, a respiratory tech, and several MA’s and CNA’s. The back office includes three full-time medical coders, three transcriptionists, and several others who are masters in their fields. The ambulatory surgical center includes three RN’s and an endoscopy technician.
Internal Medicine Group is working towards recognition as a Patient Centered Medical Home. While the practice style of each member of the Group is individualist, the first priority of the group is the provision of high-quality care for each patient. Patient Centered Medical Home provides a methodology of evidence based medicine so that the highest quality of care is provided to each patient in the practice. Another of the practice’s strengths is the group’s support in the development of “cutting-edge” diagnostic and therapeutic modalities. Each physician has joined the group with special interests and training. Over the years, IMG has gained the reputation as a leader in the state and community for introducing new technologies and services to Cheyenne and Wyoming. Among the technologies pioneered by IMG are gastrointestinal endoscopy, endoscopic ultrasound, and bronchoscopy. The Group takes pride in nurturing the growth and development of individual physicians’ practices with financial support and equipment.
The overall status of IMG is excellent. IMG strives to attain best practice metrics as published by the Medical Group Management Association. Days in accounts receivable is consistently below 35. Overhead is approximately 52%. The incomes of the partners are consistently above the national average.
The practice currently uses Centricity, General Electric’s practice management and electronic medical records software. IMG’s Ambulatory Surgical Center uses Olympus hardware and Endoworks software. There is integration throughout the clinic with personal computers, thin clients and wireless tablet PCs. As one of the first practices in the state to implement an electronic medical records system, in 2002, IMG is leading the way in both unidirectional and bidirectional interfaces with other providers. Currently bidirectional interfaces are built with Laboratory Corporation of America and Unipath. A number of unidirectional interfaces with entities such as Cheyenne Regional Medical Center and Cheyenne Radiology Group allow for fast turnaround of diagnostic test results and hospitalization information.
During the first year, a physician is considered an employee of the organization. Partnership is offered at the beginning of the second year with a very reasonable buy-in. For the new physician, the annual salary is flexibly, being similar to current partners, with an opportunity to receive bonuses based on a productivity formula. Also, the new physician is entitled to the same fringe benefits as partners including malpractice insurance, health insurance, vacation, CME time, moving expenses, and professional licensing.
